W-9 Information and Form

 
All Referees Must Submit IRS W-9 To Receive Payment

Referees are independent contractors with the Fairview Soccer Association.  In order to properly comply with IRS regulations and to ensure the accuracy of FSA’s records, we require the following procedure for providing us with your personal information:

1)   All referees must submit a W9 form with your name, address and Social Security Number.  For new referees, this form should be submitted to FSA as soon as you take the class for certification.  Failure to have this on file with FSA will result in our inability to pay you!  We recommend adding your home and work phone number for verification as well.  

2)   Any referee who has submitted one W-9 need only re-submit a W-9 to update any information that was submitted on the prior W-9.  Normally, this would be your address, phone number, city and zip but could also be your Social Security Number if it were to be different than the one originally submitted.

We highly recommend you keep a dated copy when you send it in.  That way you will have a record of the last information you provided us.  Unfortunately, FSA is not staffed to receive calls to change or verify your information.  As you complete the form, you must print your full name as it appears on your social security card, print your complete address, city, state and zip, check the “Individual/sole proprietor” box, and enter your social security number.  Please be sure that all of these fields are completed and legible.  Ignore “Business Name” and  “List Account number(s)".  Don’t forget to add your phone numbers in an open area. 

To obtain a current W-9 form, please go to the IRS website (www.irs.gov).  Here you can specify the W-9 form in the “Search Forms and Publications” field and download a current W-9 for printing and submission to CASL.
